The Migraine Tracker will encourage your patients to learn more about their migraines by tracking and discussing the pattern of their migraines.
Your patients can record their migraine triggers, pain intensity, pain duration, and effectiveness of treatment each time they experience a migraine. This information may prove helpful as you and your patients work out a migraine management plan.

Migraine Trigger Chart
The Migraine Trigger Chart will help your patients learn about common migraine triggers.
Even though the actual cause of migraines has not been determined, certain foods and physical/emotional situations may help set off, or trigger, migraines.13 Using this chart will help your patients learn about their individual triggers. Identifying their migraine triggers may prove helpful as you and your patients work to develop an appropriate migraine management plan.

MIDAS Card
The MIDAS (Migraine Disability Assessment) Card will help you determine the impact migraines have on your patients’ lives. The card was designed to give you more insight into how disabling migraines are for your patients so you can provide the most appropriate treatment. Patients should fill out the MIDAS questionnaire and then discuss their responses with you during their office visits.
